The Definition of Personal Information

Personal information is defined as "information that can be used to identify a specific individual". This Site handles the following information when registering, changing, or discontinuing use of the facilities listed on the Site.

The Source of Acquisition

Personal information is obtained from the person who has it. Within a reasonable scope, only the personal information that is deemed to have been provided by the person in question will be acquired and stored.

The Method of Acquisition

Personal information shall be obtained from the person who has it by mail, fax, e-mail, or file transfer system.
As a rule, file transfers shall use a system built and managed by the Outsourced Executive Office Operator, and encryption must be used for the transmission route.

The Utilization and Scope of Use of Personal Information

The utilization of personal information will be limited to implementing the aforementioned purposes, and it shall be used only within the scope of your prior consent.
In addition, the scope of use for the acquired personal information within the organization will be limited to the Operation and Management staff.

Storage

Personal information obtained will be stored in a secure environment in accordance with the Nagoya City Personal Information Protection Ordinance (Nagoya City Ordinance No. 26, April 1, 2005) and the Nagoya City Information Security Ordinance (Nagoya City Ordinance No. 41, March 31, 2004).
Whether stored in electronic or physical form, your personal information is kept in a location accessible only to Operation and Management personnel. It cannot be removed from that location, nor may it be copied.

The Deletion and Disposal of Personal Information

When the purpose for which the personal information was obtained has been fulfilled, we will promptly stop using it and delete or dispose of it in an unrecoverable manner.

Requests Regarding Personal Information

Only from the individual concerned will Nagoya City accept requests for disclosure, correction, deletion, or cessation of use of the personal information obtained and stored by the City in the operation of this Site.

By following the procedures for requesting disclosure of personal information, you may request disclosure of your personal information stored in this Site. This site acquires and stores important information as specified below. The information is managed by Nagoya City Health and Welfare Bureau and its entrusted contractors.

Regarding Access Logs

Four types of access to this Site are automatically recorded and stored: the IP address of the access source, the access time, the request contents, and the response results.
Access logs are used to survey user trends, to improve the operation of the Site, and to maintain the system.

Feedback Submissions

This Site has a feedback system that makes it easy for you to submit your ratings and opinions. We keep a record of all the information you submit.
We collect this information to help us research user trends and improve our operations.
